Short fiction, probably named ‘Wormholes’

I read this when I think I was about 9, around 2011. Pretty sure I remember it being named ‘wormholes’ in yellow text on purple and blue background— I’ve searched it and found the one by Dennis Meredith, but reading reviews that one seems far more rooted in science. This one read like a children’s or young adults fiction.
The plot centred around at least two people travelling through time and space to get to other dimensions. The most notable scene I remember is how the sister of someone, potentially the main character, was disabled and in a wheelchair on Earth. In one dimension, they were transformed into whales on a planet of, well, talking whales, and I think she decided to stay there, or at least very much did not want to go back.
I’ll look into Meredith’s book more, but the whale society scene is definitely the book I’m looking for. I don’t think it was very long, either, just a short paperback.
